Dean's Certification Forms
The Dean of Students Office processes “Dean Certification” forms most commonly for the following reasons:
- Law Schools
- Medical Schools
- Semester at Sea
For Transfer Student certification forms and all others please contact the Records and Verifications Office at 801-581-8968 or in person at the Student Services building, window 19 on the second floor.
To process a Law School, Medical School or Semester at Sea certification form please complete the “Applicant Information” section on the form, SIGN the appropriate signature line and submit it to the Dean of Students office by one of the following means:
- In person: Union Building room 270 (second floor, south wing)
- By mail: 200 S. Central Campus Drive, Rm 270, Salt Lake City,
UT 84112 - By fax: 801-585-5114
If mailing or faxing please include a cover
sheet with the following information: - Full Name
- UofU student ID number (or SSN)
- Current phone number
- Dates of attendance
- Special handling information (if required)
