Temporary Attendance Adjustment
The University of Utah wishes to provide temporary adjustments for students who are concerned about attending classes in person because they are a high-risk individual, live with a high-risk individual, or another COVID-related reason.
If seeking temporary adjustments due to concerns about quarantine, self-isolation,
or exposing another vulnerable individual, please complete this form.
