Dean's Certification Forms

The Office of the Dean of Students processes “Dean Certification” forms, most commonly for the following application processes:

  • Law school 
  • Medical school
  • Learning Abroad/Semester at Sea

We also complete the Transfer College Report portion of the Common Application.

To process any of these forms, please complete any  “Applicant Information” section included on the form, SIGN the appropriate signature line and submit it to the Dean of Students office by one of the following means:

  • In person: Union Building room 270 (second floor, south wing)
  • By mail: 200 S. Central Campus Drive, Rm 270, Salt Lake City,
    UT 84112
  • By fax: 801-585-5114
  • By email to deanofstudents@utah.edu
    Please include the following information with your request:
  • Full Name
  • U of U student ID number
  • Current phone number
  • Dates of attendance
  • Special handling information (if required)
